#336 Merge pki-setup into pki-server

The pki-setup contains scripts to setup PKI servers. So it would make sense to merge pki-setup into pki-server (similar to pki-deploy being merged into pki-server). In the future if the scripts are no longer needed they can be dropped from pki-server.
