#542 Remove all "respawn()" logic from "pkispawn"

With the advent of http://pki.fedoraproject.org/wiki/Upgrade, make the following changes to 'pkispawn':

  • remove the "-r" option from "pkispawn"
  • remove the "respawn()" method from the AbstractBasePkiScriptlet class
  • remove all "respawn()" methods from each and every deployment scriptlet
