#416 Fix various AsciiDoc issues

  • Changes headers up a level to ensure each page has one h1 header
    • Fixes browser tabs showing Untitled
    • Fixes strange horizontal line at the top of pages
  • Remove empty pages and references to them
  • Change formatting of linking xref to a different repository
  • Remove extra table of contents in modules/developer_guide/pages/getting-started.adoc

Everything is laid out in each commit
