#613 Don't send notifications to disabled users or hosts

Some categories of users should be omitted from notifications:

  • disabled users (status=BLOCKED)
  • the users of build hosts

I'm on the fence about user groups. I expect in most cases they should be omitted, but I expect someone, somewhere is relying on them not to be.
